In the arm with wacky inflatable arm dude hanging from the ceiling, there’s a row of children’s pictures. All the eyes are bloodied except the kid on the far left. Curious as to why.

There's no concrete answer that I'm aware of. If you explore a bit further, there is a caretaker crying in front of portraits of the same kids. Coincidentally, the portrait of the boy that you are referring to has been knocked down onto the ground.
