I'm on NG+, have filled out two Phase 5 slots and the Extreme Potential Achievement is not unlocking. Can't find anything online about this.

Before going to NG+, I had the heart is pounding prompt for humanity, could pick up the hotel cat and everything and still couldn't pick up the Golden Lie from the portrait. I've googled and read so many different comments about this final weapon and people having vastly different game-states (I read someone had gone to NG+ 5 trying to get it to work).

I'm hoping that going through NG+ I'll be able to pick up the Golden Lie, but honestly if the other achievement is bugged then I might not continue.
