The Cursed had already fulfilled her vow of vengeance. She had a bunch of combat-relevant items but I don't remember which ones. I'd statted her up as a hero-killer so mostly might, maybe some command.
She and the the first Chosen One (CO1) each had 3 minions. I don't recall if CO2 had managed to hire any minions yet.
She was the attacker definitely for CO2, I think both times.
I remember thinking after CO1 that maybe it had to be her that made the killing blow and I hadn't watched closely enough to see if it was her or a minion, but it was definitely her that killed CO2.
After the achievement didn't fire the first time I waited for the new Chosen One to pop up and sent her after them pretty much immediately -- she may have lain low first.
Let me know if there are additional details you want, but I'm not sure what I did differently.
Hmm, odd. I'll try a few things with those details in mind, hopefully the bug can be tracked down before the next patch.
As far as I can tell the code shouldn't care about that. It's going through and checking the person's traits each time, so if the UI says they have the Chosen One it should be good. Not saying it's NOT that, but it's possibly related to the CO attacking The Cursed, rather than the other way around. Hopefully bugfix soon.
