Begins at enemy level 101 then ends near 199.
Chest tiers as rewards for random loot are the same likewise.
If anything - it benefits to have a character that has reached max skill points if only to min-max the survival potentials while farming this dungeon (and this can be accomplished during any NG+).
I just tried for the first time and I died on wave 10. Looks like it worth opening the chests just after beating wave 9 in my case as I noticed the last wave is much more hard and long.
I did pick up a lot of level 100 - 105 set pieces and weapons playing the NG+ games, but it takes a lot longer, because, after all, you are playing all of the quests and traversing over all of the maps all over again. However, the extra dungeons and mini-instances make it more fun and/or interesting to level up to 100 than trying to do all of it from Mapworks maps alone.
