Presumably you're running this on a laptop? I ask since the first thing that's reported is that it's running on the onboard Intel HD graphics. Sometimes on machines with multiple selectable GPUs (such as a high and low performance one), the game isn't correctly identified to be run on the high performance GPU. As such, you'll need to manually set which GPU the game uses. In case you need them, instructions are here: http://www.pcadvisor.co.uk/how-to/pc-components/how-set-default-graphics-card-3612668/
If that fails to work, there is a bit of a hacky fix. In Steam, right click on Chronicle -> Properties and click the Set Launch Options button. In the box that pops up, enter "-force-d3d9" (without quotation marks). It'll force the game to run in DirectX9 mode, but be aware it's not officially supported. There may be a few very minor lighting or texture issues.
I tried the first measure and looks like it worked. By now, I've played 5 consecutive match and the game didn't crash :)
