Never had that issue... Slow poke internet and you load in several mins later when colossus is scaled to 4 man and they have to 3 man it and died just enough times?

I don't know I arrive, and it said that, me arrived and another person I see the damages that is on the report, around 40 K damages, I use public sessions to gather up people.

Before you were not able to join to any kind of mission that was already started but players complained until the devs gave in and made adjustments so people can enter a little late, they did not said the exact time but from what I have observed its like up to 30s, now that together with having a slow hd and/or slow internet can make things like what you described to happen.

You probably were slow to join, fight started, other people thought you were afk and left, the colossus killed you 3 times while you were loading and you know the rest.

Well something definitely slowed your arrival at the fitght for enough time to have everyone else present die 3 times and loose the fight before you joined.

you joined mid match and all 3 revives were probably used thus failing the match before you loaded in, it happens. ill equipped people can killed in seconds of the match starting.
