Help: Reserve Achievement

I have done everything that is needed for the Reserve achievement, i even hear the jingle for completing a puzzle yet i do not get the achievement, i have done this 3 times in different playthroughs from the beginning and none of them gave the achievement

I'm not sure what I can say here other than look at Comfortably_Numb's guide to see if you have done absolutely everything. But I do know that in this area there are actually two times that Mark makes a comment that you have done the right thing and you get the sound effect. You know a "there you go" and "that ifxes that" kinda thing.
