Game crash when loading level (On death, reloading save in-game etc)

My game crashes every time I die. Is there a fix for it?

Please use the Steam facility to verify your game files.
Check to see your graphics drivers are up-to-date (use Windows update and check 'Optional Updates').

If those don't fix it, reduce the game's graphics settings to minimum and die. Did you crash ?
