i found a way to sorta bypass this, i made a new character and completly skipped the tutorial, and went and bought a baseball bat, i still dont have fist but at least i can play :)
You need to add -game Unofficial_Patch to activate the fixed game. Check that the version info is displayed on the options/gameplay page! Also a baseball bat is fine as a replacement for the fists but you need to have hands to move objects otherwise you will get stuck latter!

Okay, then the patch fix finally triggered :)!
On the second character I got the message that fists had been added to the inventory, but I also checked "Skip intro" for that one
