Did you install the patch linked in the pinned thread, and enabled it? What patch version are you on?
The game requires the unofficial patch on modern systems to run properly, but you also have to enable it in steam launch options.
I can't comment on your ability to run a simple executable file, then clicking "next" until the patch installs, that's your skill issue, but the patch is always safe. Any antivirus trips are false positives, and wesp explained at lenght why those happen a few times.
The patch is safe to download from modDB, install and use. I've been playing with the patch for years, and never had a problem and neither has anyone else. Otherwise those forums would be full of posts about how someone's PC got infected after installing this patch.
If you want to be paranoid, that's ok, you're free to do so, but don't spread the baseless accusations around.
Either play with the patch don't, but he patch is safe, and GOG thinks so too since they include the patch with their version of the game by default. They wouldn't include the patch with the game if it wasn't safe, they can't be seen spreading viruses on their store.
If that doesn't put your mind at ease, then just don't play the game, or try to play it without the patch. I wouldn't recommend it because last time i tried it it was near unplayable, but you can try.
