option settings are reset on each game launch
As title says, I've been living this frasturating problem ever since my introduction with this game. I've the steam version but had to move my folder out of Steam, directly under Program Files, to avoid automatic updates (which is steam's recent forced change on all games - to not to able turn off automatic updates anymore which is plain stupidity !)

What I've tried untill now;
- My cfg files status are non Read-Only as of now (tried doing the opposite as well, didn't make any difference)
- Run the game as administrator (same as above)

Using Unofficial Patch 9.2 but this problem should not be related with the patch, since Wesp also is aware of my problem.

So any help would be greatly appreciated since this problematic situation discourages me to play the game badly :/

In-game options... From keybindings to subtitle, anything.

Was there a problem? There haven't been any updates for this game since like forever.

This has been discussed with you multiple times in the past, veleron. Including a couple or three weeks ago. Some people, for whatever reason and no matter if you say it shouldn't happen, have their Steam auto-update the game no matter what they have their updates prefences set to and this wipes out any patches or mods they have installed on the game. The only way to avoid it is play in Offline mode or do like this guy did and move VtMB out of their Steam folders so Steam can't find it to screw it up again.

I asked if there was an actual problem, because some people read [old] guides and see that they need to disable automatic updates, yet they can't find this option and then assume it's a problem, while it's not.

The Unofficial Patch usually installs an autoexec.cfg (i'm not sure if the vanilla game does). I am interested to know if your autoexec.cfg is being overwritten,if not, have you tried adding your keybindings to it?

A fresh UP 9.2 autoexec.cfg should look something like this:
cl_smooth "0"
cl_obfuscate_daylight "0"
alias detailfade "cl_detailfade 0"
alias detaildist "cl_detaildist 1200"
alias heal "use item_g_eldervitaepack"
alias afterbloodhunthealingscript "blood 1"

These values are exactly the same as of yours. However, my friend was able to send his own cfg files, and after overwriting them over mines fixed this frasturating problem totally. Now the game also remembers my recent choices I do over his settings. And the files are not even read-only, damn weird ain't it ?
This fixed the problem as I said, but still wonder what was causing my actual problem before that aheh. Anyway, we are on the road again hopefully.

Maybe there was an invisible character or something in your config files which messed them up.
