Eyes always looking at top right corner

How to reset or fix the eyes? my avatar keep looking at the top right corner no matter how many times I reset to default or turn option to always look at camera. even when tries using another avatar its still the same.

Is this avatar a custom one, made by you or an artist? If it's a default avatar from the list of 3D ones (or even 2D), let me know which one it is.

a custom one. for the 2D one i tried using Yutaka & Yutaka P and its still the same.

*Edit.
Its fixed by reducing the look at camera ratio but when look at camera ratio is full its always looking at upper right corner instead of looking at camera
