Do you have sword abilities equipped? Are they the ones that boost your sword damage, like Muscle Memory and Precise Blows? If you don't, equipt them.
Do you use Hanged Man's Venom? Do you use Thunderbolt potion or any of the decoctions like Water hag decoction? If not, then use at least venom and Thunderbolt, because they will increase your damage by a lot.
Do you have any Witcher gear? If you are sword-focused, you should use either Cat School set, Bear School set or Wolf School set.
Additionally, you can use Chernobog runestones and install them into your sword, Greater Chernobog runestone boosts your damage by 5%, three of them will boost it by 15%.
And of course last, but not least, be sure that you attack appropriate enemies with appropriate weapon - humans, humanoids and beasts are fought with steel, monsters are fought with silver.
If you do all of the above, your sword damage will increase greatly.
The DLC's, in my experience, definitely ramp up the difficulty and require a more strategic approach: oils, spells, mutagens, suitable synergy, etc., even when in the base game I rarely needed to use them to get by
