Some quest's can automatically fail, if u progress to far in the main story.
TW3 is such a huge game with so much content being hidden events, quests, encounters, that it is pretty much literally impossible to see and do everything in one run.
I’m currently on my 10th run, having spent now nearly 2k hours with this game, and I still (!) find the odd encounter and event I’ve never seen before.
So stop obsessing, and most importantly stop spoiling yourself by using quest tracking mods on your first ever playthrough.
Just enjoy, learn, and use that for the next run… yes, it’s a major time investment, but it’s worth it.
Heh, yeah, I know that problem myself... but TW3 luckily permuates on more than just what the player chooses in the dialogue, also when you go where, at what time of day, at which point of the story, etc pp.
Like I said, pretty much impossible to even remotely see everything, do everything in a single playthrough.
but there are quests, such as "gangs of novigrad", which are bugged leading to failure. this is a timed quest, but only if you know about the bug can you definitely prevent it.
is that a choice you make, the game being faulty in this respect?
don't tell people how to play their games.
good job spending 2,000 hours on this masterpiece, but not everyone has that availability of time
