Occupying armies of a defeated faction make provinces inaccessible
When a faction is defeated its remaining armies/territory normally become rebels. However, I've encountered a glitch where an army which is in the process of occupying an enemy province when the capital falls will retain its affiliation to the defeated faction. This means that anyone who wishes to invade the province with said army must first declare war on the defeated faction (I haven't tried a faction that doesn't follow the Code of War yet). However, declaring war doesn't work because the game keeps giving you the message "you can't break the Code of War, you have to wait until next turn" regardless of how many turns pass. Such provinces also have a resistance counter that never diminishes. It ends up being an effectively inaccessible province that nobody can annex.

I play as Ravengard and this too has happened to me, quite a few times actually, but to answer your question attacking the stack of the defeated army, you can beat them and take the province, but you are stuck in a state of war for the rest of the game, I was unable to finish my last man standing due to this fact, or the minor victory campaign, most annoying, as a result I have to play power games in order to win a game.

Okay, so you can actually attack them if your faction doesn't follow the Code of War. That helps somewhat, but as you say it basically makes certain game modes unplayable. Thanks for the information!

No problems, this issue has been giving me grief for a while, it and a couple others bugs, it breaks my spirit.

I encountered this issue too. Basically, you end up with a bunch of provinces on the map that's controlled by rebels but you can't attack (because you can't declare war on the realm).
