3. Not really, It was happening a lot in the Warehouse Heist like everytime so I had to stop trying to do it.
4. I don't recall there was a button that said "ok" I'm pretty sure and when I hit it it closed the game but if I tried to tab back into the game it wouldn't let me.

Also, I've never gotten a crash log from any of these crashes.
I had this happen to me too once, but have yet to figure out an exact repro case to help catch it. I might try and make a debug level with a hundread guys locked in rooms to see if that causes it. One report was from just picking up the note in hideout... this is super hard, but we're diligently trying to find the cause.
And it *seems* it's related to where on the map you are. So far, I was always clearing the right border of the map / screen.
I've got the impression the game has problems detecting this area of the map. Most of the time I will even get the bubble pointing at one of my goons displayed directly above his/her head when (s)he is standing near the edge of the map and in full view. Moreover it may be impossible to directly select one of the gangsters in that area. Happened at the arcade when I was preparing to 'storm' the back room and now kicked me out of the Fly's mansion while I was checking the garden at the right.
Maybe it's related to aspect ratios? The guys from L.A. Cops reported problems with 16:10 resolutions using the Unity Engine. On the other hand The Masterplan seemed to be pretty solid when it comes to res detection and display, even when it was in EA.
Still, this can be pretty frustrating and I hope you hunt the bug down ;)
Like before, I was trying to select one of goons standing very close to the edge of the map and *boom* null object crash.
Were you able to reproduce the issue?
