Look through all of your addons. See if you have anything that would cause that.

I used a Hl2 Beta weapons pack and it started appearing. I already deleted and unsubscribed to it yet it still happen. I haven't tried it without mods yet but I'm about to. (note that I already uninstall and redownload at it still happens so it's highly probable the cause is my mods)

Disabling should be just as good as uninstalling.

Found it. It's the HL2:MMoD sound replacement. Oddly enough it's a SOUND replacement. Don't know what happen with that but it ain't good.
