This happens in campaign mode, and at least in the tower trial, during the Loan Shark encounter. When i turn down the loan shark's offer, and i refuse to trade in my Worker's Hammer, the item won't properly come back in my inventory. I can only see it when i try to vendor it at the camp, and it disappears for good as intended if i do sell it for 5 gold.
