see if your graphics card cranks up to boost clock or stays at idle
see if your CPU is running at 100% for some reason
disable full screen optimization (right click the .exe, properties)
There are also a couple options in the 'General tab' for options.
'Physx GPU acceleration' and 'Physics Multi-threading/. Play around with those, might be an issue there.
Under the 'Graphics tab', make sure it is set to 'fullscreen' and turn the 'disable full screen optimization' ON for the games .exe, like da_nibbest said.
Make sure in the 'Graphics tab', select D3D9 for 'Device'. It is the very first option.
Try off any other game 'overlays' for any other applications you are running, like Discord. Having multiple overlays turned on at once can cause a lot of issues.

as other have said please make sure you have no Directx overlays running with NS2 and your anti virus solution isn't treating ns2 like a malware. Both can cause serious performance issues. E.g. I have seen the MSI Gaming tool bringing ns2 down to 24-26 fps on high end specs.
If you are sure such software is not running at your pc try to follow these steps to provide us with a performance log:
The msi gaming software was just an example. There are various DX overlays causing similar issues with ns2. Another common offender is razer's synapse DX overlay.
Maybe the game isn't using your GPU?
other than that, in nvidia control panel, manage 3d settings, program settings, if you cant find NS2 in the dropdown, add it manually, power management mode set to "prefer max performance"
I've had problems with that in the past.
If you do, try and enable Raw mouse input in the games controls. That was always how I fixed the issue with games that gave me issues.
Other than that, I can't think of what else it could be.
Playing both, with msi afterburner and razer synapse ns2 fine for years.
Also you haven't mentioned the OS, so I assume it's a modern Windows x64 OS
