Hmm, at least on my end, using the link SleepingSeaweed posted indicates to "clear the stage without taking damage", as they mentioned. However, I did check another guide (Dark_Cecil's walkthrough from GameFAQs, which is the one I usually refer to), and it does say the criteria is to "Complete the stage a second time". Have you tried doing it without taking damage as well?
