Complete mission without dying broken

I was able to complete the task for killing demons at current or higher level but it even though i am not dying it is not filling the task for not dying on my ancient relic item

This is a challange? Or a heroic deed? Which task might this be specifically? :)

This was for a red glowing item in my inventory. it was for the skull banner item. It was the challenge thing that unlocks the extra stats underneath the normal item stats.
I got it eventually but it was definitely not after 3 missions it took about 15-20
