Glitched enemy placement
In random dungeons it can happen that enemies that you need to kill to proceed and are immobile until approached (like burnies) spawn in places where you can't approach them in any way (like on top of a tree in Maggy Temple) meaning that your dungeon run just became impossible to complete. Did you have a ton of useful items for the boss? Kiss the run goodbye.

I've never had this happen to me, sounds sucky though, maybe carry an ice grim with you? since the icicles spawn at the top of the map and fall down you might be able to stand underneath them and cast it?
