Other than that, props for you taking the time to make that thread.
"Night Shifts require On-call rooms for the doctors, and Nurse's stations for the nurses. They'll be found and be buildable once you can enable Hospitalization."
The On-call rooms and Nurses stations are used to staff the operating theatres. Wards, HDU's etc and therefore are for both day and night shifts.
You unlock the Surgery Department by building it. As I have mentioned on #6, you only need to build the rooms with numbers that are red. For clinics it's typically a waiting room and an outpatient clinic and that'll make the department viable. But again, check under Management mode which rooms are red, those are what you need to build.
Oh sorry if it came off as meaning that on-call rooms and stations are exclusively used by the night shift staff. I only meant that to enable night shift, On-call rooms and stations are a prerequisite.
Nah dude, you're not stupid. The game has a pretty poor way of saying whether or not a department is available or not. I was prety much like you when I started playing lol, I thought there was gonna be a big pop saying "You can build Sx dept!" and waited thinking I progressed far enough to build a new department, but nah that didn't happen. I did feel like an idiot however and had a "WTF seriously?" moment when I realize that you can actually build all departments right from the get go lol.
I think I'm stuck there. I'm playing in sandbox mode and have to treat 50 patients to unlock the next department. At the moment I have 3 insurances unlocked which send me 40 patients, 2 bonus patients from an unlock and if treatment takes longer some more when the patients leave the clinic the next day. My best score was 47 patients with no untreated ones.
As all other departments are still locked and I have to build another departement to unlock an insunrance which would give me more patients to unlock the next feature I can't do anything more now. Any ideas what I can do about that?

None of the departments are locked. Just build some of the buildings in red. Overnight is locked. Surgery itself is locked, but the outpatient is unlocked.
Of course the clinic stuff is available.
Thanks for giving me the hint that they aren't locked.
There could also be value in having a night shift of janitors even if you have no overnight patients, for example.
4/5 definately consider this FAQ and if you find the price a little high wait until a holiday sale.
And last question :) Is it possible to add more than 200 employees in sandbox mode?
Thanks!
