There are a lot of Adults on Steam.
But I agree that there's tons and tons of adult stuff on steam and it'll be really hard/impractical for steam to remove it all.
But you bring up a good point.
You all are making this harder then it actually has to be. All they need is one more flavor of Steam gift card. One that requires a brick and mortar to verify your age, and once you spend the card on the store nothing will be hidden anymore. That and hire a couple actual human beings to double check an automated system. That is tasked with finding profiles that are against the rules.
Steam has pretty good and functioning tools to prevent "children" from seeing adult games. Anyone with a semblance of IT literacy can use them.
By "filth" you mean sexy-looking video game characters who wear clothing more revealing than a Hazmat suit.
As for the gift cards, unless the merchants system flag for an age check, nobody is going to do it. Lord knows how many kids used to buy albums with explicit lyrics because the person ringing them up didn't check for the warning sticker.
