Nobody is hacking anyone's Steam account. At some point in the past you either used your login on a 3rd party site, or logged into Steam through a phishing page masquerading as the proper login page. The 2FA through either the Steam App or Email means that even with your login and password, there's almost no chance they will be able to guess the code in the short time that it's usable.
That's not enough. Follow all the steps in post #1.
You have to find out how someone keeps gaining access to your account.
Is it just your Steam account that is being "hacked"?
No Steam will not replace/return items once they have left your account regardless if it was you or a hijacker who did it.
