the public bathrooms in the big mall downtown near the soup kitchens here are dirty as hell, I find a bush outside before using them, there are three soup kitchens within 5 blocks of each other here in Ottawa

This is all just a question of legislation. There really isn't any reason to make a big deal about it. The law in the UK is changing so that public toilets will offer universal toilets, which are self contained single person toilets that can be used by any gender. Each one has a toilet, and a sink. They will normally be provided in addition to standard toilets, so people don't have to use them if they don't want to, but they can also be provided instead of standard toilets if there is limited space.
I mean, this is not new. Such toilets have been available in doctors surgeries for many years.It's not rocket science and it's not necessary to get all upset about it.

It depends on which country/location and situation. If you are really "busting" then a lot of people can excuse the intrusion. However in certain countries, there are legal and cultural restrictions.

I think in parts of Europe and here in Australia, if you simply announce "apologies but it's an emergency" then people won't care as much.
