You're a kid on Little Saint James who broke out of his cell and is now desperately trying to escape while on the run from security and Epstein's patrons. With some skill and luck, you might be able to disarm and kill a guard for their gun, and use your new firepower to make an easier escape, get back at those who did this to you, or maybe even save some of your fellow abused. The bolder you get the more heat you attract and the more security ups its game on killing you. Maybe include some survival elements, like add a day-night cycle for security and patron activities / routines and let the player get hungry/thirsty/cold/tired in the meantime as a way to force them into buildings to recover
