All maps, every single wave on every single mission. (6 maps, 6 missions, ~40 waves).

You need to beat every wave from start to end for it to count, you can always check progress in-game by using command !tt_mission and !tt_tour

Just like any medal, it is untradable, unmarketable. You should be able to change name/description of it, though.

You ask for announcing your contest/tournament/thing/etc. on their blog in community tab, and they will.

Wave 666 is pretty hard to test so most people just decided not to risk it (even though we never stated minimal amount of waves for TT).
Hillside, we had one mission for Hillside but it didn't end up good enough to be top6 (even though if I could make "unlimited" winners I'd let it through just for "the gimmick")
