Clarification about the boss: every 3ish seconds it fires a pinpoint accurate shot from it's cleaner's carbine. It both knocks players back a bit and marks them for death (hence the icon)

After the 5 minute timer ends, there's still a grace period of probably 10-15 seconds before the pain comes and makes everyone sad.
