Throughout Grand Theft Auto 4 you will stumble upon some situations in which you can go either one way or the other. There are some people that want to know the outcomes of their future decisions, there are some people that have already played and want to see what path they chose, and there are some people that are simply curious. Thus, I have created a guide with the decisions and their outcomes.

I can't stress enough what a huge SPOILER this guide is, so, if you do not want to know about things that will happen throughout the plot, don't read this guide.

Decision 1 - Ivan the Not So Terrible
Mission: Ivan the Not So Terrible

You can either:

1. Kill Ivan. This meaning he will not appear again in the game.
2. Spare Ivan. This meaning he will appear later in the game and give you a side-mission.
Decision 2 - Ruff Rider
Mission: Ruff Rider

You can either:

1. Kill Cherise. This meaning she will not appear again in the game.
2. Spare Cherise. This meaning she will appear later in the game and give you a side-mission.
Decision 3 - Holland Nights
Mission: Holland Nights

You can either:

1. Kill Clarence. This meaning he won't appear again in the game.
2. Spare Clarence. This meaning he will appear later in the game and give you a side-mission.
Decision 4 - The Holland Play
Mission: The Holland Play

You can either:

1. Kill Playboy X. If you choose this option you will get Playboy X's house(and all that comes with, like pool table or parking spaces), you will get Dwayne as a friend.
Note: If you become good friends with Dwayne, you will be able to call him and ask for back-up(he will send two guys to help you).
2. Kill Dwayne. Choosing this will get you 25000$.
Decision 5 - Blood Brothers
Mission: Blood Brothers

You can either:

1. Kill Francis McReary. No major outcome. I think that at the "Undertaker" mission, the road to the cemetery is easier. Not sure though.
2. Kill Derrick McReary. This way you will be able to call Francis McReary one time to get rid of the wanted level.
Decision 6 - That Special Someone
Mission: That Special Someone

You can either:

1. Kill Darko Brevic. No outcome.
2. Spare Darko Brevic. No outcome.

* The dialogue you have with Roman will be different but it doesn't change the plot.
Decision 7 - The Decision
Mission: The Decision

You can either:

1. Take the deal. If you choose this you will kill Dimitri and get 250000$. However, Roman will die and Kate will leave you.
2. Take revenge. By choosing this Kate will die, but you will kill both Dimitri and Pegarino.

One big note about blood brothers is if you call Francis when him and Derrick meet up, you can get him to pay you more for taking out derrick.

I always go for Revenge. I don't really care about money and besides. Roman lives!!!!!

I can't let anything happen to Roman. I will always pick the Revenge ending since Kate was kinda useless, but Roman and Niko have a bond that will never be broken, they are The Cousins Bellic.

Regarding the Holland Play,you also unlock a unique outfit if you choose Playboy X and I think you can only equip it in Playboy X's home.
