Introduction
Icewind Dale was released June 29, 2000 and finally got achievements added to it (available and unlockable in beta December 8, 2017) courtesy of Beamdog's Enhanced Edition. This guide was written before the release of the 2.5 patch, so it is possible there have been changes or bugs introduced since then. If you find any bugs, report them to Beamdog via Service Desk https://beamdog.atlassian.net/servicedesk/customer/portals. Like other guides, this guide contains spoilers.

There are a few things to consider as you are building a party. If you're going to achieve 100%, then you'll have to do several playthroughs for achievements such as beating the game solo, on Story mode, and on Heart of Fury mode. Also you'll want to optimize your party for the level 30 class achievements, so plan what classes your characters have accordingly. You'll also probably want a class capable of Grand Mastery, and a mage that can learn both Contact Other Plane and Malavon's Rage. I recommend doing three playthroughs to get 100%:

1) Standard playthrough, use a full party of six and just enjoy the game. Get what achievements you can because not all achievements can be done on Story mode it seems, but also focus on leveling up your characters and getting them good equipment.

2) A speed run with a single character on story mode. Just do the bare minimum with your character. You cannot die on Story Mode, so you can be as reckless as you want. Focus on getting any other achievements you may have missed.

3) A Heart of Fury playthrough with your party from the first game, who should all be a pretty high level at this point. Playing through on HoF is significantly easier if your characters are maxed out.

The Blade of Aihonen achievements are also noteworthy as you will need to obtain a broken sword at the beginning of the game, restore it at the end of the main game, and then carry it into the expansion, so read over those achievement descriptions before leaving Easthaven.

Also be aware that some of the achievements that require you to kill a specific NPC may not unlock if a monster or animal you summoned got the kill. If it doesn't unlock, try reloading and making sure one of your party members gets the final hit.

********IMPORTANT********
Additionally, many of the achievements are coded to only unlock if certain requirements are met ON YOUR LEAD CHARACTER. If an achievement does not unlock, try moving the character who met the requirements into the top spot of your party.

If you find any errors or typos, let me know. If you find any bugs, report them to Beamdog via Service Desk. Any feedback is appreciated.

Heart of Winter Story Related Achievements
The Heart of Winter expansion can be accessed at any point by talking to Hjollder in Kuldahar, who will lead you to the expansion areas. Be aware that you cannot return until you've finished it. Alternatively, you can start the expansion from the main menu and bypass talking to Hjollder in Kuldahar. You will also want to bring the Restored Blade of Aihonen with you into the expansion. These are the Heart of Winter (HoW) achievements that cannot be missed by playing through the main story.

The Final Resting Place
Visited the Burial Isle.

The Gloomy Mountain
Visited Gloomfrost.

The Frozen Sea
Visited the Sea of Moving Ice.

Dragon Slayer
Killed Icasaracht, the White Dragon matriarch.
Unlocked upon killing the final boss.

Hero of the Frozen North
Completed Heart of Winter.
Unlocked shortly after killing the final boss.
Class Related Achievements
Grand Master
Obtain Grand Master weapon proficiency level.
There are only some classes capable of Grand Mastery. According to Baldur's Gate wiki, these are Fighters (any kind), and Archers (Ranger kit; long bows, short bows, and cross bows only). Fighters can unlock this at level 9.

You can actually just start a Heart of Winter game (which will start you at a higher level) with a Fighter and you should be able to achieve Grand Mastery immediately.

Oathbreaker
Become a Fallen Paladin or a Fallen Ranger.
Pretty easy achievement, just create a Paladin or Ranger, save your game, and start killing innocent people. Your reputation will take a nose dive and you will become a Fallen Paladin or Ranger.

(UPDATE: Looks like the character must be in the top slot in order for this to unlock; this may change later, but they are looking into it).

The Raging Mage
Memorized Malavon's Rage.
You can obtain this spell by killing Malavon in Lower Dorn's Deep. All you need to do to obtain this is memorize the spell (level 7 Mage spell), ready it in the spellbook, and rest to have it available for use.

Song of the Sea
Sang to the Sea Spirit Elisia.
This achievement requires you to sing to Elisia in the Prologue. You will need to have a Bard speak to her and you will be given the option to sing to her. You will be given an option to ask her to teach you her song; do this, finish the conversation, and the achievement will unlock.

Epic Mage
Reach level 30 with a Mage or Sorcerer.
See the info for Ultimate Warrior.

Epic Rogue
Reach level 30 with a Thief or Bard.
See the info for Ultimate Warrior.

Epic Warrior
Reach level 30 with a Fighter, Ranger, Paladin, Monk, or Barbarian.
See the info for Ultimate Warrior.

Epic Priest
Reach level 30 with a Cleric, Druid, or Shaman.
See the info for Ultimate Warrior.

Living Legend
Reach level 30 in two classes with the same character.
See the info for Ultimate Warrior.

EDIT: Some people have reported problems getting this achievement on a triple class character. You may want to have a separate character with just two classes to get this one to unlock.

Ultimate Warrior
Reach level 30 in three classes with the same character.
This can be done as a multi-class Fighter/Mage/Thief and will require A LOT of XP. This does stack with the previous level 30 achievements, so getting a Fighter/Mage/Thief to level 30 in all classes will unlock Epic Mage, Epic Rogue, Epic Warrior, Living Legend, and Ultimate Warrior (some people have reported that Living Legend did not unlock for them, so you may want another character to dual-class for that). Other than that, these achievements are straight forward. Just get to level 30 and they'll unlock. Playing on higher difficulties awards more XP, so you may want to consider that when going for these. Additionally, humans are unable to multi-class, and dual classing only allows a character to have two classes, so you will need to do this on a non human character.
Difficulty Achievements
Master Tactician
Complete the game on the Heart of Fury difficulty setting. Before starting a new game, go to the options screen and change the difficulty to Heart of Fury and keep it on until you finish the game. This is essentially hard mode.

Storyteller
Complete the game on the Story Mode difficulty setting. Like the Heart of Fury difficulty, Story Mode must be enabled before starting your game and then left on for the rest of the game. Story mode is basically super easy mode; your characters don't die and are all easily able to defeat enemies. It is a good idea to combine Story mode with your solo run while using a Fighter/Mage/Thief. This makes it easier on you and also makes it so your character gets all of the XP, instead of splitting it with party members.

It is possible that some achievements cannot be obtained on Story Mode. Constructed Betrayal, for example, has been reported to not work on Story Mode. It is possible this is the case for other achievements as well.
Alignment Achievements
All of these achievements are very easy to obtain. You simply need to create a six-person party that all fall in the same alignment area. The achievement should unlock as the game starts. You can even pre-generate a character that is either good, neutral, or evil and then create a party with six copies of that character to unlock these achievements.

Company of Champions
Have six party members who are good-aligned.
These alignments are Lawful Good, Neutral Good, and Chaotic Good, so a full party of characters with this alignment should unlock the achievement.

Order of Guardians
Have six party members who are neutral-aligned.
Characters with Lawful Neutral, True Neutral, and Chaotic Neutral.

Band of Villains
Have six party members who are evil-aligned.
Characters with Lawful Evil, Neutral Evil, and Chaotic Evil.
Base Game Quest Related Achievements
Liberator
Freed all slaves in the Wyrm's Tooth glacier.
There are two sets of slaves in the area: a group at the aquatic museum and a group at the Frost Giant caverns. At Wyrm's Tooth Glacier you will find a frozen aquatic museum filled with Frost Salamanders. Beneath it is a group of escaped slaves led by a man named Gareth. They need a key to escape, and the key happens to belong to the Frost Salamanders' leader, Kerish. Obtain the key from Kerish (by killing him if need be) and return it to Gareth to unlock the achievement.

You will also need to free the slaves in the Frost Giant caverns. To do this, you will need to speak to their leader, a man named Davin. He'll give you a quest to kill a Frost Giant named Gorg. Before doing so, however, clear their escape route of enemies (Yetis, Trolls, Black Ice Knights, and Kontik). Go back, kill Gorg, return to Davin, and you should be good to go.

Constructed Betrayal
Turned Malavon's Iron Golems against him.
During the main story you will be searching for six badges carried by six lieutenants. Malavon is one of these lieutenants and he can be encountered in Dorn's Deep. Prior to confronting him, you can find a Book of Golem Building in Marketh's room, which informs you that if you speak a key word then you can control Malavon's golems. You will need a character with high Intelligence to be able to do this, and they must be in the top spot of your party when speaking with Malavon. When you encounter him, speak the command word and turn them on their master and this achievement will unlock.

NOTE: There have been reports that this cannot be obtained on Story mode so plan accordingly.

Giant Slayer
Killed Joril, the leader of the Frost Giants.
At the Wyrm's Tooth Glacier, you will be searching for six badges carried by six lieutenants. Joril has one of these badges, and you have to get it from him as part of the main story. There are two ways to do this: you can talk him into giving you the badge, or you can kill him and take it. Killing him will unlock this achievement for you, but persuading him will not. You can also persuade him to give you the badge and then kill him after, which gives the most XP.

The Secret Garden
Restored the Arboretum to its former glory.
This quest can be started by speaking to Valestis on the fourth floor of Corellon Tower in the Severed Hand. He requires you to collect various items for him in order to restore the Arboretum. None of these are immediately available and will require you to progress further in the story to obtain. The items required are as follows:

-Barrel of Pure Water: Found in the Frost Giant caves (Wyrm's Tooth Glacier) in a container by Kontik, a follower of Auril.
-Cage of Squirrels: Purchased from Nym in the Gnome village (Lower Dorn's Deep).
-Cage of Exotic Birds: Purchased from Nym in the Gnome village (Lower Dorn's Deep).
-Seeds: Found in Malavon's Dome in the Artisan's District (Lower Dorn's Deep). The seeds are in a container in the study.

It has been reported that you can complete this achievement with just 3/4 of the items. It is possible that the conditions require the cage of birds OR the cage of squirrels, though both can still be used.

The Reborn Blade
Acquired the Restored Blade of Aihonen.
This achievement is easily missable! While you are in Easthaven during the Prologue you need to talk to the fisherman named Jhonen. He will tell you about some strange dreams he's been having about a mysterious woman. Offer to investigate for him. Follow the lake left until you find a blue skinned woman. She will tell you a story and give you a broken sword to give to Jhonen. Return it to him.

Far later in the game (practically the end), you will return to Easthaven. You can once more encounter Jhonen (at the same place you met the woman in the Prologue), who will tell you about how the blade you gave him at the beginning of the game has been restored. He will give you the Restored Blade of Aihonen to use for the final fights and the achievement will unlock. Keep this sword for your playthrough of the expansion.

The Unseemly Relic
Found Pale Justice, the Holy Avenger sword.
This achievement is tricky because it has been changed from how it was in the original Icewind Dale. A cut quest has been restored in order to obtain this sword, and it needs to be done in a particular order in order to be successfully completed.

When you enter Lower Dorn's deep you will be attacked by The Voice of Durdel Anatha. He cannot die at this point. If you 'kill' him, he will simply collapse to the ground for a little bit before coming back. Go ahead and walk away from him. This quest is about killing him for good. Your next goal is to meet the drow merchant Nym in the hidden gnome village (unlocking this area involves stealing a sack of potatoes for a gnome named Tarnelm). You can talk to Nym to learn a bit about how he is involved in this story.

The next step is to go to the temple (just past the palace courtyard) and meet Brother Harken. Harken will summon the voice again. You will need to tell the voice what you learned from Nym. The voice will dissipate at this point. You can collect his bones and return them to Kuldahar to finish his quest, but that part isn't necessary for obtaining Pale Justice.

Now go to the Artisan's District (the place with Malavon's lab/house). There are a couple of small houses to the left of Malavon's place. An Umber Hulk should now break the wall and attack you. Kill it and go inside the hole it just made. You should find a single body that you can loot. Pale Justice will be on the corpse.

There is also a workaround/exploit for this achievement. You can actually start a playthrough of Heart of Winter using the default party to unlock this. The Undead Hunter, Cadriel, who is part of the default party has this item equipped, so the achievement will unlock upon starting the expansion.

Black Swan
Acquired the Black Swan Armor.
This achievement is pretty close to unmissable. While hunting for the six lieutenants' badges, you will encounter one of them named Maiden Ilmadia. You will need to kill her and take her badge to progress the story. She also carries the Black Swan Armor, so as long as you remember to pick that up while you're getting her badge then this achievement will unlock.
Heart of Winter Exclusive Achievements
The Lone Wolf
Discovered the true identity of the Lonelywood werewolf.
To start this quest you will need to talk to Emmerich in the Ranger's Cabin in Lonelywood. He will tell you a story about being attacked by a wolf and about some trappers who moved into the area. You can continue with the main quest at this point, but periodically come back to Lonelywood and check on the trappers. They will start dying off as you progress through the story. After two of them have died you can question Emmerich about the deaths. If you either saw the werewolf yourself (it can be found in town during the night), or learned that it is magical, you can conclude that Emmerich is indeed the werewolf. You can then visit Kieran Nye in the Whistling Gallows Inn to get an amulet to help Emmerich keep his lycanthropy at bay and complete the quest.

Glacial Efflorescence
Brought a rose of ice to the bard, Murdaugh.
Murdaugh can be found in the Whistling Gallows Inn in Lonelywood. Talking to him will start the quest "A Rose for Murdaugh." You are tasked with finding him a rose made of ice. Tiernon, a dwarf in the Gloomfrost Glacier who makes weapons out of ice can help you with this. Speak to him, explain Murdaugh's plight, and he will make a rose for you. Return to Murdaugh and give him the rose and this should unlock.

Allurement
Visited the ruined castle in the Anauroch desert.
This achievement is unlocked by speaking to Hobart Stubbletoes in the Whistling Gallows Inn in Lonelywood (Heart of Winter). Talking to him will start the Trials of the Luremaster questline and unlock this achievement. It should be noted that actually starting the quest means you have to finish it before being able to return to doing other things.

Resisted the Lure
Completed Trials of the Luremaster.
See the description for Allurement. Completing that quest and defeating the Luremaster will unlock this achievement.

The Singing Blade
Acquired the Singing Blade of Aihonen.
You must have first completed the achievement "The Reborn Blade" for this to be unlocked (so see the details of that achievement first). You must carry the Restored Blade of Aihonen +5 with you into the Heart of Winter expansion and speak to Tiernon to upgrade it. Once it is upgraded, the achievement will unlock. Tiernon is a story related NPC so you should find him as you progress through the story. Doing the Heart of Winter expansion before finishing the main game of Icewind Dale can cause you to miss this, as the Restored Blade of Aihonen can only be obtained in the final chapter of the game.

Infernally Vexed
Acquired the infernal Vexed Armor.
This armor/quest can be found in the lower levels of Burial Isle. You will encounter a suit of animated armor called Vexing Thoughts. You need to be careful while talking to him otherwise you could miss this achievement. After you've spoken to him (do NOT say that you will command him), cast the 5th level Mage spell Contact other Plane (you will need a mage who knows this spell). A Planar Spirit will appear and you can ask them for Vexing Thoughts' true name, which is Chalimandren. After you have the name, talk to Vexing Thoughts again, speak his true name, and command him. This will give you the Vexed Armor and unlock the achievement.

Cold Fury
Assisted the dwarven smith Tiernon to forge an ice weapon.
Tiernon can be found in the first level of the Gloomfrost caves, and you will need to speak to him to progress the story. There is an option to ask him what he is doing in the Gloomfrost, and he'll explain that he's there to make a weapon out of ice. Offer to help him, and he'll give you a choice of an item to make. Pick which one you want, help him, and the achievement will unlock.
Miscellaneous Achievements
Flying Solo
Complete the game with only a single party member.
Pretty straightforward, but could be difficult. Maybe do this simultaneously with your Story Mode playthrough to make it easier on you. It appears that you cannot start a game with a party, kick the party out at the end, and then finish with just one character, so you'll have to go the whole way with just one character.

Godlike
Increase an attribute score to 25.
In order to get this one you're going to need to get a character with high stats and try to raise them higher using equipment or consumables to get bonuses.

The easiest way to pull this off is by using certain potions in game (red potion and violet potion) that will set a stat to 25 temporarily. If you find either of these, you can just drink them to unlock the achievement. There is a Red Potion in the Hall of Heroes, and another in the Frost Giant caves. If anyone has the location of the Violet Potion, let me know and I'll update the info.

Untouchable
Reduce Armor Class to -15 or lower.
Dexterity is your main stat for determining natural Armor Class, so pick a character with high Dex for this. This can be accomplished with equipment, spells, and potions. There are plenty of lists online of equipment that boosts AC but if you just gather them as you go, you should have enough. I used the following on my 17 Dex character:

Bathed-In-Blood AC: -1 AC
Boots of the Fox: +1 AC
Great Shield +3: +4 AC
Illian's Hunting Helm: +2 AC
Black Wolf Talisman: +1 AC
Battle Axe +2: Defender : +2 AC
and Potion of Mind Focusing (gives +3 Dex and stacks, so these are invaluable for this achievement)

The Cleric spell Defensive Harmony also gives a bonus. There are plenty of AC boosting weapons/armor in the game, so you mostly just have to collect them and find a group that works together.

Juggernaut
Reach 150 Hit Points.
Constitution is the main stat for determining HP, therefore your easiest shot at getting a high HP value comes from Dwarves and Half-Orcs, who can both get 19 points in Constitution from the beginning. They should also be classes such as Paladins, Fighters, or Rangers, to get the most HP. Since Icewind Dale has a much higher XP cap than Baldur's Gate, you should have no problem getting characters to this point later in the game. If you have any trouble with it then try using items to increase Constitution or base HP (there are potions that are helpful with this, so just hold onto any that give these if you're concerned or impatient). By the end of my playthrough (on insane difficulty for max XP), I had three characters over 150 HP. This could also be different if you don't play with the max HP on level up option (default set to on).

Destroyer
Killed 1,000 creatures.
Pretty straightforward. If you want, you can find a low level area and grind this achievement on weak enemies, such as goblins. I had no problem reaching this by the end of my first playthrough however.

Loremaster
Obtain a Lore skill of 100 or more.
A character's lore skill increases with each level they get, though some get more than others. According to the Baldur's Gate wiki (BG uses the same engine as Icewind Dale), Bards gain 10 lore per level, Thieves and Mages gain 3 per level, and every other class gains 1.

Intelligence and Wisdom also provide a lore bonus so increasing these stats using potions and the like can help you reach 100. A table to the values given can be found here: http://icewinddale.wikia.com/wiki/Wisdom

You can actually just start Heart of Winter (this will start you at a higher level) with a bard and you should have enough Lore for this to unlock.
Conclusion
Hopefully this guide was helpful. If you notice anything that is wrong with the guide, please point it out so I can get it fixed. Thanks to everyone who has contributed. Good luck hunting!

Grand Master seems to be another one that only works when it is the party leader - simple fix though. I just put my archer to the top, and got it.

@Torimir
Either because they implemented achievements as I was already part way through the game or because I was playing in offline mode. I don’t remember which.

How come almost half of your achievements ( link ) were unlocked at the same time (Unlocked 9 Dec, 2017 @ 11:06am)?

Untouchable can be done fast if you start a fighter and the default priest in Heart of Winter, I believe fighter starts with -11 AC which you can easily up to -15 with Defensive Harmony
