I'm lost! What do I do?
Shadow Warrior's maps can be pretty confusing at times. Go to http://www.3drealms.com/sw/walkthroughs.html if you need to figure out where to go in a map.
The Grenade Launcher is your best friend

The Grenade Launcher is easily one of the most potent weapons in your arsenal. Grenades can bounce off of walls and have a large blast radius, making the launcher great for wiping out enemies before they can get a good shot. Hear some enemies in a nearby room? Bounce a few grenades near a wall to wipe out anyone in the room. About to enter a new room? Toss a grenade and see if it hits anyone. Ran into a room and there's a lot of Uzi-wielding enemies waiting for you? Leave the room and bounce grenades at them from a distance. You'll know a grenade has killed something when you hear a SQUISH sound. Beware though, bodies blown up with an explosion will also make a squish sound.

Once you know how to use the Grenade Launcher, you'll find that it is a lot easier to survive because you can defuse situations without enemies being able to attack you.
Blow up suicide bombers
If a suicide bomber is killed normally, his ghost may appear from his body. However, if you gib him, the ghost will never appear. Blow him to pieces with the Rocket Launcher or Grenade Launcher to save yourself some trouble.
The Heart weapon is awesome
The Heart weapon is extremely powerful. When used, it will spawn in a copy of you. Your copy is armed with a Railgun and is extremely aggressive. The copy can attack with the Railgun much faster than you can. Even a group of Rippers, the ape-like monsters, will be torn to pieces by your copy in a matter of seconds.
Cut down weak enemies to save ammo

The katana can kill weak enemies, like the Uzi-wielding guys in tan pants and small gorilla monsters (Baby Rippers), in one slice. Not only will slicing them up save you ammo, it is also very fun and satisfying.
Need ammo? Look for secrets.
Secrets are filled with ammo and health. They can give you the firepower you need to make difficult situations much easier. If you need a little boost, http://www.3drealms.com/sw/secrets.html has a guide that shows every secret in every level.
Rockets rock!
The Rocket Launcher is a great weapon. A single rocket will take down a majority of enemies in one hit and is quite effective against the Red Serpent and Sumo enemies you'll encounter later in the game. Make sure you always have a few rockets with you at all times.
Kill Shadow Ninjas first
The Shadow Ninja guys are jerks. They can blind you with a flashbang and can shoot fire spells that will often kill you in one hit, no matter how much health you have. Kill them first to save yourself a lot of trouble and quickloads.
Don't underestimate Sticky Bombs
Sticky Bombs are surprisingly effective because of how dumb the enemy AI is. Simply throw a bomb or two near the enemy, then get away from the bombs. Because the AI is stupid, enemies will happily walk into them without a second thought.

This is particularly effective against the suicide bomber enemies. Lob a few near the bomber, then make him see you. When he spots, he'll charge into one of the bombs, setting off a chain reaction that'll turn him into mush.
When in doubt, Nuke 'em
The Nuclear Missile is the ultimate weapon for taking down large groups of enemies quickly. One missile can clear out an entire group of enemies in one shot. Launch the missile and hide behind something before the missile hits something. The rocket will obliterate the entire group. However, if you don't get behind cover right after you launch a rocket, you will get nuked as well.

To switch to the Nuclear Missile, take out the Rocket Launcher and hit the “switch to Rocket Launcher” key until the nuclear symbol on the gun is lit up.
Watch out for stray enemy Sticky Bombs!
The Female Warrior (the girl in a bikini that has a crossbow) can toss a sticky bomb. However, you may not notice it when you're fighting groups of them. After taking them down, listen for a “beep”. If you hear it, be careful, because a sticky bomb is nearby!

Here's an example of a stray Sticky Bomb. If you charged the Female Warrior coming for you, you'd probably be blown to pieces.

For me - the ONE thing that keeps me from playing ....

I got some other tips:
#1: Camp a non-gibbed suicide bomber with your katana ready. If a ghost emerges, you can 2-shot it before it can move.

#2: Ghosts are weak to your katana. Unless they're completely invisible for that 1 whole second, you can just run up and hit them. If you connect even 1 swing, an Uzi will finish them off right away.

#3: Gib Guardians and Rippers whenever you can! Always do it. ALWAYS.
Guardians are these tall, green-skinned beasts that first appear in the 1st level of the full version of the game. They have a dagger, and shoot energy orbs out of their eyes. Gibbing them has a chance to drop their relatively intact head, which can be picked up as a powerful weapon.

Two riotgun blasts and a rocket should gib them, or two direct rockets/grenades.
No, you can't gib their corpse to acquire the head, nor merely kill them for it.

Rippers are the guide-aforementioned black gorillas. They first appear at the end of the 2nd level in the free episode of the game. The big orange ones also count, and they first appear midway through the 4th level of the free episode. Gibbed, they may drop their still-beating heart. It's exceptionally potent as a weapon.

Like with Guardians, weaken Rippers with a couple riot gun blasts, then finish off with a rocket.
They, too, must be gibbed alive.

#4: Explore alternate fire modes with the weapons!
The Katana, Uzi, Riot Gun, Rocket Launcher and Guardian Head are such weapons. Simply press the same # key multiple times to change their function.
The Katana switches to fists, which heal you from punching target dummies. They deal more damage when you're cloaked by a smoke bomb.
You can dual-wield or solo-wield Uzis this way.
The Riot Gun becomes either accurate semi-auto, or inaccurate burst-fire.
The Rocket Launcher can be set to heat-seeking missiles, or firing nukes.
The Guardian Head cycles between a plasma gun-like function, an expanding ring of wall-penetrating bolts, and a piercing row of flame (like the Shadow Ninjas do.)

#5: Use your items!
Trust me, use them, there's usually 2 of any given item in every map.
--It might seem overkill to be invisible with the Smoke Bomb, until you are facing a couple dozen guns or get wrecked by a Shadow Ninja's fire spells. Also, it supercharges your fists.
--Flashbangs... don't seem useful. Might be like a baby Smoke Bomb, or maybe it disables ninjas from using special abilities.
--The portable medkit is best for dodging medkit pickups near full health, or before fortune cookie pickups, or when in a tight spot. Be careful not to waste portable medkit pick ups though.
--See a fork in the road? Take a path, find enemies, toss a gas bomb, and take the other path.
--Caltrops are weak, so use them to hear enemies flanking you.
--Night vision highlights enemies through fog, so use it in thick fog or if you suspect there's a Shadow Ninja in the dark.
