You forgot the secret area in Level 3 - Inner Sanctum. Looking at the background squares made up of light blue dots, if you count 12 going right (5 going left) and 3 going upwards (12, 3), you'll see the path that goes downwards, then left, until reaching a dead-end. At the dead-end, look up and you'll see an area not listed on the map. On that square I mentioned earlier (12, 3), in the downward path, push against the left wall.
That secret room contains 3 of the yellow circle collectibles and a jet-pack.
