Defuse the bomb, then die to the monster.

To defuse the bomb, you first need to find the phone. Its spawn is randomized but I always found it near the five notes. It will glow at the distance, so shouldn't be too hard to spot.
Note: The notes aren't required for anything.

Interacting with the phone will give you the code required to defuse the bomb.

Find the briefcase (also random spawn, but I found it most of the time near the lake) and input the code to defuse it.

Now, go find the monster and die.

Use the same steps posted in the previous achievement, but escape this time.

Escape zone is your car, which is located behind the fence right at the starting area (just follow the road).
Note: You can't escape until you have defused the bomb.

Defuse the bomb, read the book at the basement and escape.

To access the basement, you first need to find the key to open the latch.
This can be found by interacting with a dead body, but its location is random like most things in this game. I found it most of the time near the five notes or outside the house.

The basement is accessible by interacting a few times with the bed, inside the house.

Once you've defused the bomb and read the book, head to the beginning area and escape using the car.
