If you spot inaccuracies/typos or have answers to my questions then please do comment. I also should be able to answer any questions unless they're related to a particular text file location.

There's a chance that I might add screenshots for hidden achievements and some extra descriptions for ID tags and audio tapes.

Statistics & Grinding Achievements: 9
All achievements here can be done at any point in the game. Some require enemies, some other require resources. If you're having hard time gathering resources then consider switching to Rookie difficulty.

Butcher
Cast off 50 enemy limbs
Regular zombies can have their heads and limbs blown off. Meta way of destroying limbs is to use a shotgun and shoot zombie arms close to point blank. Liev's submachine gun and all handguns loaded with enhanced ammo are also great for dismemberment. I assume head pops do not count. I'm not sure if destroying limbs on dead zombies counts either, needs confirmation from the community.

Bullseye
Take out 50 enemies with headshots
Simply shoot regular zombies with any weapon at the head and every lethal blow will count.

Boxer
Knockback 20 enemies using melee attacks
If you press LMB/shoot button without aiming then you'll perform a melee. This melee is used to push back regular zombies, but it also works well on the first miniboss. The idea is to execute the melee attack when a zombie is trying to grab you (or is recovering for your shots/ is idling). I assume it's possible to get this achievement by pushing the same zombie over and over again. Using melee consumes stamina. One and two handed weapons have different melee animations.

Quick hands
Utilize 100 fast reloads
One of game's most important mechanics is reloading. Assuming you have a spare magazine with at least 1 round in it tapping the reload key without holding it will perform a quick reload and drop gun's magazine on the floor. Don't use inventory reload if you're going for the reload achievements. Alternatively, you could sit through a boring grind if you don't plan to play the game more than twice. Goes without saying, but you need a weapon that utilizes a magazine, like a handgun, submachine gun or a desert eagle.

Take your time
Utilize 100 slow reloads
Same concept as for the achievement above, except that instead of tapping you should hold the reload button. Gun's magazine will be placed back into your inventory automatically.

Free hugs
Free yourself from 20 grabs
Self explanatory. Regular zombies have only one attack and that's grabbing. When you're grabbed you're supposed to mash spacebar to free yourself. If you mash fast enough on Rookie 1 grab takes <10HP and on Daymare <20HP. You're very likely to get this achievement naturally.

Chemist
Craft every type of rapid use fluid
Best time to do this is closer to the end of the game when you've stocked up on resources. There are 3 types Rapid Use Fluids: Health, Stamina, Mental. You create them by mixing Hexacore Additive (H-Additive) with Energy Drink, Epinephrine, Mental Drops respectively.
Here are all possible combinations which you need to create and mix:

Health

Stamina

Mental

Health+Health

Stamina+Health

Mental+Health

Stamina+Stamina

Mental+Stamina

Mental+Mental.

Crafting all of these yourself nets you the achievement.

Trader
Perform 10 exchanges at an H.I.V.E. terminal
During Chapter 2 you'll find your first H.I.V.E. terminal, it's unmissable and each upcoming chapter has a couple. Terminal's purpose is to item store, item trade, ID tag trade and save. To get this achievement you need to perform item trades (second tab). My recommended trades are:
  • x15 handgun bullets for 2 H-Additives
  • x15 enhanced handgun bullets for 30 regular handgun bullets

Addicted
Reach 50% of overdose
Best to be done along with Chemist. All you need to do is use multiple Rapid Use Fluids in a row until you get the achievement. I believe on Daymare the Overdose meter goes up faster.
Missable Achievements Part 1: 23
These can only be triggered either once or only a couple of times in the entire game. These are listed in a chronological order by chapters. These are spoiler-heavy.

CHAPTER ONE

Jurassic theft
Shoot the shaving cream can
As Liev you have an objective of retrieving the Pollux sample. You complete a nitrogen puzzle and then defeat a miniboss. Pollux sample is in Chamber 04 and so is the cream can. It's to the left of the entrance/sample, on the floor. Shoot it.

CHAPTER TWO

What is IT?
Shoot the red balloon
As Liev you get to go through series of streets and houses. Once you go through second house by solving the 637 lock puzzle and leave the house you eventually end up on a street with a very prominent red balloon above a curb drain. Shoot the balloon to get the achievement and check the drain for extra reference appreciation. If you reached the 3rd house then you're locked out from getting it.

Wrong turn
Find the arrow in the tree
As Sam once you leave the tower you go down a path. Keep looking on the left and you'll see an abandoned campsite. One of the trees next to the site has a couple of red arrows sticking out of it. Examine them to get the achievement. If you reached the bridge then you overshot and need to go back.

File still open
Find the branded oak barrel
As Sam you find a shotgun. Once you leave pig pens you'll be in an open area with 2 zombies. To the right there's a tractor and right next to it there's a barrel with a knife and a scribble. Examine it to get the achievement. If you've entered a house then you overshot.

Stabbed
Find the movie poster
Condition: As Sam in the hospital you've given your first chore and two supplemental items, one of which is an override cable.
Go to the previous room you just came from and hack the door. At the end of the room, to the left of the computer there's a green SHOUT poster with a knife in it. Examine it.

Maverick
Find playing cards
Condition: As Sam in the hospital.
Right after doing the achievement above you head to the west section of the hospital through a specifically long corridor, where you have a brief hallucination. Going through the double doors puts you in a H-shaped section. There are 4 rooms which can be explored, the one we're looking for is close to the end of the current hall, past a wandering zombie, to the left. There are cards next to the bed, examine them.

Daymare syndrome
Shoot at 5 false enemies
Spans across multiple chapters. As Sam in the hospital you get introduced to false enemies. Once you fetch the ID tag for the doctor and return though the specifically long corridor you'll encounter another hallucination. This time a zombie attacks you. Shoot it. It will disappear. Now there are more than 5 of these in the whole game and there's already a couple of them in the hospital. Stay on your toes and use your handgun to probe enemies. Only Sam gets these false enemies. I believe being grabbed or using melee does not count towards this achievement. You're likely to get this achievement during your first playthrough.

Wrong melee
Perform a melee attack on the Melted Man
Caution: this achievement can kill you. Manually save in case you're also going for a no death run.
Condition: As Sam in the hospital after you've completed the objective of your 2nd chore by initiating archive self-destruction.
Go back to the parking lot. You get introduced to the Melted Man. Simply walk up to monster and melee it. There are more monsters in the game, but this is one of the best places to get this achievement.

Hijacker
Hack into a "very hard" terminal
Spans across multiple chapters, but can be done on this one.
Condition: As Sam in the hospital after you've started the 3rd chore, used closet key and picked up pincers.
Once you leave the closet you get ambushed by 3 zombies. Get rid of them. It might not seem so, but they opened you a new room by breaking out through the previously sealed double doors. Go there and you'll find a hackable locker. This is the first Very Hard terminal in the game, there are more than a couple. Successfully hack it and you'll get the achievement.

Gunslinger
Get the magnum
Condition: As Sam in the hospital after you've used pincers.
The area you've unlocked has a corpse with a magnum and magnum ammo. Pick up the magnum to get the achievement.

CHAPTER THREE

Invader Studios hires
Find the programmer's corpse
As the chapter starts Raven wakes up in Invaders Studio apartment. Leave the room, go down the corridor to the right. There's a room with lots of computers and a corpse in the chair. Examine the corpse the get the achievement. It's also possible to get this achievement in Chapter 4 as Sam.

Green thumb
Find the chemical formula mural
Condition: As Raven you're in downtown and have solved the electric water puzzle and reached Hexacore save room.
Down the street past the BOOKS store there's a small playground. Check the right wall that has a V-JOLT formula to get the achievement. It's also possible to get this achievement in Chapter 4 as Sam.

Problem solving
Find the barrelled corpse
Condition: As Raven you're in the sewers and have went up a long spiral staircase.
Once you're on the top of the stairs take out the zombie, pick up ammo next to the ladder and go into the corridor. Turn right at the crossroad and you'll end up in a room with a lot of barrels. Examine the open one with a corpse to get the achievement.

Rat sterminator
Find the sewer pinball
Condition: As Raven you're in the sewers and have deactivated system lockdown.
Go through a sturdy looking door, take out 2 zombies and end up in a crossroad tunnel. To the left of you is a closed gate, to the right is a path with stairs and in front is a path that turns right and leads to a room. Go into that room, take out the zombie and examine the Sterminator 2.1 pinball machine to get the achievement.

Back in time
Find the flux capacitator
Condition: As Raven you're in the outskirts and have restored the power.
Go down the hill back to the car service, press the button to open the shutters. You'll see a DeLorean and a switch next to it. Right next to the switch there's a table with a flux capacitor surrounded by orange cables. Examine it to get the achievement.
Missable Achievements Part 2

Hand cannon
Get the desert eagle
See the achievement above for directions. Press the switch to raise DeLorean and under it you'll find an officer's corpse with the desert eagle. Pick it up to get the achievement.

Déjà vu
Wait for the suggestion
Condition: As Raven you've reached dam entrance.
Once you reach the entrance you'll have to input your own ID code. Enter a random code and that should trigger the achievement.

CHAPTER FOUR

Blown away
Find the exploded corpse
Condition: As Sam you've reached Hexacore's save room.
If you go up the long string of stairs then you'll end up in the wrecks of a gas leak explosion. If you check the middle of the room then you'll spot a corpse. Examine it and you'll get the achievement.

Where's the fog?
Find the dead viewer
Condition: As Sam you've reached Black Jack pub.
Go though the BAR and you'll end up in a back alley with a Melted Man. To the right of the monster there's the entrance for Cheryl Hotel. Enter it and go upstairs. Take out the zombie and enter room that is closest to the stairs. There's you'll see a dead familiar person in a single seat sofa watching TV. Examine him to get the achievement.

Primitive experiments
Find the science magazine
Condition: As Sam you've reached the radio tower.
In front of the radio there's an open area and a closed gate. Right next to the gate there's a small shed. That shed has locks for the game and the science magazine we're looking for. As soon as you enter the shed examine the right shelf to get the achievement.

CHAPTER FIVE

Games of war
Find the tris game
As soon the chapter starts turn around and check the small monitors. Go to the rightmost one. Examining it will give you the achievement.

4th of July
Find the researcher's corpse
Condition: As Raven you've met Mason.
Once you regain control look to the right and you'll see a big window. If you go to the middle of the room you'll notice that there's a corpse stuck to it. Examine the corpse to get the achievement.

Codename Broken Arrow
Find the H.A.F. plate
Caution: this achievement can kill you. Manually save in case you're also going for a no death run.
Condition: As Raven you've met Mason and went past the HIVE save spot.
You'll find yourself in a dark tunnel. Escape sequence from Liev starts and you open 3 doors via QTEs. Shoot the zombie in front of you, turn around and when Liev is next to it shoot the gas tank to the left. Run up all the way to the elevator and activate it. Go back in the tunnel and get ready to shoot the closest gas tank to the left. When it's done run past Liev and turn right. This is the part of the tunnel that has 4 gas tanks. Check the middle of the right wall and and you'll notice a sealed door. To the left of the door there's a plate. Examine it and you'll get the achievement.
Achievements for Collectibles & Secrets: 5
The following achievements require extra attention. I suggest using dedicated text/video guides from other users to successfully collect all the necessary items.

Surgeon
Get all ID Tags
ID tags are collected from particular corpses. First ID tag that you get is unmissable, but the rest are. Once you collect all of them you'll get the achievement. The last one is located in Chapter 4, in a secret room in the outskirts.

Bookworm
Get all documents
You're supposed to collect all files in the game. You can use your DID's 4th tab to check your progress. All 3 characters have separate files. The last files in the game are in Chapter 5, dam facility offices.

Disc jockey
Get all audiologs
Same as with ID tags and documents there are audio cassette tapes. Collecting all of them will give you an achievement. The last tape is in Chapter 5 right after you talk to Mason.

Hound
Find all Hexacore secret rooms
The game teaches you to find three first secret rooms and they're unmissable. Finding the rest isn't an issue either if you pay attention to sound. Last secret room is in Chapter 4, outskirts. Entering all of the rooms will get you an achievement.

Hunting season
Spot all puppet deer
Throughout the game there are deers hidden around. They produce squeaky and slightly static noise. Shooting all 20 of them will get you an achievement. If you pause the game you can see how many deers there are in this area and if you have found them.
Completion Achievements: 3
These are challenge achievements. You unlock them by following certain conditions that apply to the whole run.

Self explanatory. You have to rush through the whole game and finish the whole thing under 4 hours. You can do it on any difficulty. On Rookie it's realistic to finish the game casually within 2 hours. Once you finish Chapter 5 you'll get the achievement.

Immortal
Finish the game without dying
Self explanatory. You have to finish the whole game without using any continues. You can do it on any difficulty. If you do die then go back to the main menu and reload the latest autosave use a manual reload. On the current build of the game even if you died after a checkpoint, reloading said checkpoint from main menu will negate the death. Once you finish Chapter 5 you'll get the achievement.

As i just did the Deja Vu Achievement. It seems like inputting a wrong code doesn't trigger anything. At least i tried a few times. Waiting at the keypad for some time triggered a voice line from Raven saying where to look for the code.

The achievements Quick hands & Take your time also work with the gun.

It should only be said that for these achievements you have to set the setting to Classic 90s Mode at the beginning of your run in order for these achievements to work

Hey, I just got back from my 2nd run. And for the documents, yes, they carry over playthroughs. I only have to pick the ones I missed on my 1st run and it unlocks. However, the log on your DID is empty, so you'll have to check your older saves to pinpoint what you miss. Feel free to add the info to your guide.

Some little notices: I think it applies to the other collectibles as well (would be weird if it didn't) but cannot confirm since I got them all in my 1st. And I play on GOG, but I assume it applies to the Steam one as well.

Don't recall anymore, maybe someone else here might remember.

Hello, I would like to ask a question about the collectibles. Do they carry over playthroughs? Like if you missed, say, 5 documents, do you only need to collect those 5 during your next run, or must you collect them all again?
