the only challenges that I have completed are hunted 1 and hunted 2 :(

It’s hard but absolutely possible. You should be prepared very well. I mean, you must be familiar with all regions in the game, you should plan the route and stick to the plan. The first two graves were the hardest, I lost like 50% of my character condition. Then it becomes much easier because you find better clothes and stuff. I also watched a YouTube video where a guy finished this challenge during 2 in-game days!.. That helped a lot.

wow just wow i have been always thinking that challange is just a bad joke from devs but its possible to complete

It took 11 days 1 hour and 32 minutes to visit all 5 graves...
