The Black Hat Lady:

Once a woman named Elizabeth, this phantom guards what was once her home, killing anyone she encounters.

Elizabeth was found dead on the 29th of March, 1996, cause of death being suicide from both hanging and cutting.

The reason for Elizabeth's suicide remains unknown, but she seems to have returned for revenge as every victim she has killed was killed in the same way that she killed herself, strangled and bleeding.

The only clues towards why Elizabeth killed herself were mad rambling notes that mentioned a entity called "the red hands"
