Using the Bonemerge Tool, Here it is. (Shoot the Ragdoll first, and then the hat, and shoot somewhere else then it automatic equips)

Try Open the Spawn Menu.
Theres a Folder called 'Addons' if you open it, you have to look for 'TF2 Ipod Shuffle with Earbuds Hex'
