Seems to work fine for me so far.
About the issue of a new Spindown Dice appearing somehow... Well, my best guess is that this was a conflict with a different mod.
As for the issue of the Void on Angel Room... Bear in mind that the game still counts Downpour/Dross I as Floor 1, so if you somehow got an Angel Room in that floor, then this might be the issue... Otherwise, I'd also guess it's a mod conflict issue.
No clue about the issues with Ascent, so I also assume it's a mod conflict issue.
Sorry for not being able to help any more than that, but I really have no clue about what could have caused the issues.
During the Ascent sequence Spindown Dice also gets removed from your inventory onto a pedestal each time you activate it. I didn't have Butter fwiw, and it wasn't spawned from a spindown attempt--ID was too low for that.
I think you can just disable/enable the mod if you want to do that though. Not as convenient as the way that Natopotato did it, but it should work.
It probably wouldn't be too hard to add this function in all honesty, but I kept on promising other people to do things to my other mods and have yet to do it because I haven't touched Isaac in over a month, so like... Well, don't expect any changes for the foreseeable future.
To do that i have to set custom starting items on for every single character every time i want to enable or disable this mod, and for more than 20 character it's a bit tedious to do every time.
The AB+ version had a general on/off flag to enable/disable all characters all at once.
