In the chapter select, there is a white dot to the left of each chapter.
This contains that chapters puzzle to complete for achievement.
After each puzzle is complete, return to chapter select.
There will be 13 puzzles to complete.

Other achievements:
Reminisce play through the prologue.
A new start play through the epilogue.
