Outfits are based on Government type and population ethics. Adds 3 outfits per ethic of the population and governor and ruler outfits are based on government type.

v3.4.5 I have no issues. Im not sure, maybe you have another body mod.

Just curious, what version of Stellaris are you running?
I'm running v3.4.5 and the game won't even recognize the SE - Human - Body mod upon start up.
