Overview
Force OpenGL 
Don't use fullscreen or borderless windowed mode. Do this instead. 
Don't plug in/turn on your controller until after you've launched the game

Force OpenGL
1. Right click the game in your library and select "properties"
2. In the general tab, paste this into the launch options: -force-glcore32
3. You're done!
Don't use fullscreen or borderless windowed mode. Do this instead.
1. Set the game to windowed mode
2. Restart the game
3. Press alt + enter to put the game in borderless windowed mode

For some reason, this increases stability for me. All you have to do is alt + enter every time you launch.
