Mission 1: "Fog of War"
Nothing but Net
Neutralize the 'Fog of War' machine gun with a frag grenade.
After launching the airstrike and clearing a small room, you will come to a rail yard where someone on your team will mention that there is an MG nest on a nearby deck. To get this achievement, you have to stop the gunner by using a frag grenade. The best method is to sneak around the left side, killing the few enemies as you go, until you can get behind the concrete staircase leading to the gun. Throw a grenade up the stairs and around the corner; it should kill the gunner and unlock the achievement. If you miss, or the grenade doesn't kill the gunner, restart the checkpoint and try again.
Mission 2: "Piccadilly"
Trigger Discipline
Do not injure any civilians in 'Piccadilly'.
This entire mission takes places on the streets of England. As the description says, you'll need to complete the entire mission without accidentally shooting any civilians. This can be a bit of a pain in the beginning of the mission because civilians are running everywhere. Just take it slow and don't fire any shots until the civilians stop running. They aren't endless, so just hide behind cover until they're all clear. Additionally, your team does a good job of killing a lot of enemies for you, especially in the beginning of the mission. Push up and stay behind cover and your team will kill most enemies for you. Once you get through the first third or so of the mission, virtually all civilians will have run away. Some are hiding or crouched though, so be especially careful with any long distance shooting. If you accidentally shoot a civilian you'll unfortunately have to start the entire mission over. Thankfully it's not a long mission, but as long as you're patient and careful it should be relatively easy to avoid shooting civilians.

Circus Tour
Kill at least one enemy while inside The Reading Place, Aural Chic and both Subway undergrounds.
you have to kill an enemy in each of the four specific locations in the achievement description. The mission is extremely short, but getting a kill in each spot can be a little tricky, due to some not having very many enemies, and the number of police that arrive to help you. Thankfully, you can always restart the mission from the pause menu if you think you screwed up.

Below is a brief overview of the best way to do this:
1. After clearing out the initial enemies and going past all the flaming cars, you will see the first subway underground to your left. Enter and kill the enemies inside.

2. Return to the surface and cross the street into Aural Chic, where you will find a single enemy holding two hostages. Kill him.

3. Exit out the other side of Aural Chic to find the second subway underground. This is the hardest part, as there will not be enemies here. You may get lucky and get one of them to follow you inside, but this is unlikely. Instead, walk through to the other side and begin walking up the stairs. This will cause a suicide bomber to run at you. Make sure you're both inside and headshot him for the kill; it doesn't matter if he explodes after.

4. All the way across the area will be The Reading Place, a two story book store. There is one enemy on the ground floor, one on the stairs, and two upstairs. Killing an enemy here should unlock the achievement if everything was done properly.
Mission 3: "Embedded"
Play Dead
Kill all the enemies in the 'Embedded' field of dead bodies
You will reach an area where Alex will say "What the hell is this" and you will see a lot of dead bodies and a destroyed vehicle. Farah will then tell you to get down as a big group of enemies comes through the door in front of you. You can get through this section in stealth if you holster your weapon and stay still while prone, but for the achievement, you have to kill all the enemies. As soon as they appear, open fire and take down all of them to earn the achievement.

Companion Block
Only use one cinder block and bring it to the end of 'Embedded'.
Near the start of mission 3 "Embedded", Farah will tell you to act as a worker to blend in. Turn to the left to see a bunch of pallets of cinder blocks. Pick one up, you now have to carry the block through the rest of the mission. Thankfully, you can always drop the block to kill enemies and progress the mission, you just have to bring this particular block to the very end of the mission. Due to the low number of enemies and stealthy sections, it really isn't hard to carry the block with you into every encounter and drop it before stealth killing anyone. Just make sure to remember which one is yours, as there will be others laying around. When you get to the ladder to the tunnels at the end of the mission, you will automatically drop the block and complete the mission, earning the achievement.
Mission 4: "Proxy War"
Ashes to Ashes
Burn 4 enemies with a single molotov.
The easiest and most reliable spot to get this achievement is near the beginning of the mission. After the cannon fires and you storm the base, you'll have to take out some enemies and move up, at which point you'll get a checkpoint. You're told to use the drone planes again, but ignore that prompt and instead run out. A helicopter flies toward you and lands on a helipad somewhat nearby. This helicopter is carrying four enemies. If you can hit the helicopter with a molotov, it'll kill all four enemies and unlock this achievement (and will unlock Wild Fire as well). Just be aware this is pretty difficult to do on Veteran difficulty, because you'll have a lot of enemies shooting at you as you try to get close enough to the helicopter to hit it with a molotov. Consider ignoring this achievement on your Veteran playthrough and replaying on Recruit to get this with ease.

Long Way Down
Crash a helicopter by shooting the pilot.
Shortly after the mission starts and you get inside the base, you'll be told to fly RC planes. Instead of doing this, go over to the trucks and aim for the helicopter hovering in the middle. It may take a couple tries to actually hit the pilot, but the checkpoint is right at the helicopter, making it easy to retry.

Wild Fire
Take down a flying Helicopter with a molotov
If you use the solution in Ashes to Ashes, you'll get this achievement naturally then. If you're playing on Veteran and can't get it there, there is an even easier spot to get this achievement. Right after the truck backs into the airfield gate to ram it open, you'll charge forward with everyone and then a helicopter will pop out and gun down a lot of people. It flies directly toward you and over you, and for some reason doesn't shoot you. This is a perfect opportunity to line up a molotov and throw it up at the helicopter as it approaches. Aim decently high to make sure you hit it. If you miss, the checkpoint is right there to reload and try again.
Mission 5: "Clean House"
Wall Hax
Save Alpha 3-2 from being downed.
You are tasked with killing all the enemies in a multi-story building. When you get to the third floor, you will notice a door on your left; if you stay back as the game intends, an enemy will open fire through the door and injure one of your teammates. To get the achievement, you have to prevent this from happening. The best method is to try and get in front of your team and open fire through the wall and door before anyone else gets to it. This will allow you to enter the room and clear it like normal. Once you clear the second floor entirely, you will progress to the third and the trophy will unlock.

Golden Path
Complete 'Clean House' without being hit using one bullet per threat
This entire mission is very short and just takes place in a single home. To unlock this achievement, you need to kill every enemy in the house with a single bullet without getting shot. This can be a pain, but you can reload your last checkpoint as often as needed until you get it just right, and there are plenty of checkpoints in this mission. Make sure you're using your rifle. It's semi-auto and only fires one bullet at a time, and will kill each enemy in one shot no matter where you shoot them.

The first room you enter has three enemies. I found it easiest to attack from the open doorway in the hall and shoot the guy at the far end of the table first, then the other guy at the table, then the one closest. Up on the next floor is a room with one guy holding a woman hostage. You'll have to make your shot count so you only shoot him and not her. Wait until she goes and grabs the gun to shoot her, or you'll void the achievement if you shoot her before she poses a threat. Then go to the back room and hit it open, and shoot the one remaining guy.

Up on the next floor, you'll have to be very quick to strafe in front of the door and shoot the guy behind it without him shooting you. Bash the door but don't enter, and aim at the door on the left to easily take out the guy that comes through. Aim right and slowly enter and wait until you can see the guy's head behind the couch to take him out. On the last floor, after the woman runs to her baby, bash the door open and immediately go prone and aim under the bed in the back on the right to quickly shoot the guy that pops out from under it. Once he's dead you're done, just go upstairs and let your teammates finish off the woman to finish the mission, unlocking this achievement.
Mission 6: "Hunting Party"
Good Effect on Target
Kill an enemy with a direct hit from a smoke grenade.
As you progress in this mission, you'll be told to use smoke grenades to blind machine gun nests. Make sure you save at least one smoke grenade though, and don't spam them. When you get to the building that your tank and helicopters annihilate, you'll have to kill a bunch of enemies to get inside. You'll then push open blue double doors and civilians will run out. Kill the couple enemies and move up the hall. Make a right to follow the hall, and there are a lot more enemies. On the left side is a room with an enemy lying on a gurney. He'll die in one hit from a smoke grenade, so throw it at him to finish him off and unlock this achievement.

Theoretically you can unlock this on any enemy in this mission as long as you shoot them so they drop to the ground but don't die. It's pretty much luck reliant for this to happen, and is unlikely. The guy on the gurney is a guaranteed kill, so it's best to utilize him.

Press [BOOM] to Defuse
Blow up 3 tripwires with explosives.
Right at the start of this mission you'll be introduced to tripwires. Instead of defusing it as told, you'll need to blow it up with an explosive. Grenades work but are difficult to time properly. Instead, equip your grenade launcher. Fire it just past the tripwire to blow it up. There are four more obvious tripwires as you progress through this mission. Save your grenade launcher ammo to use on these tripwires until you blow up three and unlock this achievement.

Unfortunately, reloading your save or restarting the mission at the beginning again to grind the first one doesn't seem to work. Just remember to blow them up and you'll get this naturally as you progress through the mission. For me, there was a decent delay until the achievement actually unlocked too, so bear that in mind.
Mission 7: "The Embassy"
Love from Above
Destroy 4 trucks with 4 drone strikes before they reach the end of their path in 'The Embassy'.
Toward the end of the mission, you will have to defend two rooftops outside the embassy from a huge enemy onslaught. During this section, you will eventually gain access to a missile launching drone, the missiles being aimed with a laser pointer. To get the trophy, you have to destroy four enemy trucks using only four drone strikes, one per truck. This portion will begin after a blue civilian vehicle drives down the road to the left, which you should ignore. At this point, the trucks will begin to drive across the area. The first will go through the middle grassy area, the second will go down the left street, the third will also go through the middle. Then, about 20 seconds later, a fourth truck will also head through the middle area.

Keep in mind that you have to lead the targets a bit, as the missiles take 1-2 seconds to hit the target. If you mess up, immediately reload the checkpoint and try again.
Mission 8: "Highway of Death"
Driver's Ed
Shoot the driver of the suicide truck.
After you're around halfway through the mission you'll be tasked with taking out mortar teams with your rifle. Shortly after doing this you'll be told there's movement back by the blue house, which is where you shot the fruit during the tutorial. A truck will come speeding from behind the house trying to reach you to blow up. What you need to do is shoot the windshield to destroy the armor on it, and then shoot the driver to unlock this achievement.
Initially the truck is really far away and swerving like crazy, making it near impossible to hit. As it gets closer, it becomes easier to hit, and there's a section when it gets closer where it drives almost straight towards you, giving you a perfect opportunity to shoot the windshield and driver quickly.

Pit Stop
Stop three APCs with Hadir's sniper rifle.
Almost immediately after Driver's Ed with the suicide truck, white smoke will show up on the left side of the screen and four APCs will roll in. You need to disable three of them to unlock this achievement. To disable one, you need to shoot at least three of its tires. Thankfully, each one has three tires per side. Line up your shots, and you should have plenty of time to disable three APCs before they reach you. If not, reload your checkpoint and try again.
Mission 9: "Hometown"
Two Birds
Kill both soldiers with one shot in 'Hometown'.
At the end of this mission, you'll split up from Hadir and he'll tell you to call him for a distraction. Head down into the field and call him when prompted. When the enemies walk away from you, get up and grab the revolver on the table near where they walked. You need to kill them both with one shot, but a shot anywhere on their bodies will kill them (headshots aren't required). Move so they're in a line and aim to hit both. If successful, the achievement will unlock. If not, reload your checkpoint to try again.
Mission 10: "The Wolf's Den"
Hang Time
Kill 3 Enemies while you are on a ladder.
you will eventually be separated from Farah. In this short area, you will have to kill a handful of enemies and then attempt to escape from a burning room. There will be three ladders here, and each one will have at least one enemy hovering on top or nearby. Simply kill three enemies while on ladders (doesn't all have to be done on a single one) to get the achievement. You can also reload checkpoints to get the kills more quickly.

Tunnel Rat
Complete 'The Wolf's Den' tunnels using only the 1911.
The first part of this mission takes place outside and in a couple buildings. Don't fret that you don't have a 1911 pistol. After going down into the tunnels and a cinematic plays, you'll regain control and have one. From here until the end of the mission, you must use no other weapons or gear except your 1911 pistol. This means no guns AND no flashbangs, molotovs, etc. Not only that, but there have been reports of the achievement not unlocking if you even pick up another gun. Using a melee on enemies is up for contention, but it's a safer bet to avoid that as well to ensure the achievement unlocks.

The main challenge with this achievement is ensuring you don't run out of ammo. If you're playing on Recruit difficulty, this isn't an issue because whenever you die or reload a checkpoint you'll respawn with full ammo. On higher difficulties, you'll need to manage your ammo. I personally unlocked this achievement on Veteran difficulty and still had 17 bullets left at the end of the mission, and I didn't get any additional ammo along the way. So there is plenty of ammo as long as you aren't spraying bullets mindlessly. There aren't many enemies in the tunnels, it's usually just one or two at a time, so just line up your shots well and you'll have plenty of ammo. There is more ammo for the 1911 at the section where the enemies light everything on fire and you have to climb the ladders, so at least if you can make it that far you can get more ammo there.

One last important thing to note is that when you get to the guy on the machine gun, you'll be instructed to use flashbangs to blind him. Obviously you can't do this if you want to unlock this achievement. Instead, wait for a lull in his fire and run/slide to the next piece of cover. Even on Veteran I was able to make it from cover to cover without being killed while he was shooting. Just make sure to sprint and slide.

Once you reach Farah at the end of the mission, this achievement will unlock.

Mission 11: "Captive" or Mission 14: "Into the Furnace"
Got Something on Your Face
Spit on Barkov.
There are two opportunities to get this achievement, and both are during conversations with Barkov. In either instance, the game doesn't prompt you to press a button to spit on Barkov. Both in the beginning of Mission 11 and at the very end of Mission 14, you'll be given four dialogue options when talking to Barkov. This is when you can spit on him by pressing the melee button even though there is no prompt to do so. Whenever you do it, you'll unlock this achievement.

Dodged a Bullet
Never get hit by the sniper while escaping captivity.
During this mission, you'll break out of the main building into an open area and a helicopter will fly overhead. As you round the corner you'll be introduced to the sniper via a red laser indicating where he's aiming. The goal here is to kill the sniper in the warehouse without getting shot by him along the way. There are many other enemies in this area, and it's fine if they shoot you, you just can't get hit by the sniper. If you do, immediately pause and reload your last checkpoint.

There are two ways to take out the sniper. The first is how the mission intends: push up to the warehouse, killing enemies along the way, and enter the warehouse and kill the sniper above you. You're a sitting duck once you enter the warehouse though, so this can be annoying to do without getting hit. An easier way is to grab the RPG outside the warehouse at the top of some steps of a construction area. Shoot the RPG up at the sniper to kill him. As soon as you hear an ally say that the sniper is dead, you're safe, and this achievement will unlock.
Mission 12: "Old Comrades"
Hot Swap
Get at least one kill with eight different weapons when completing 'Old Comrades'
Note that this achievement will not unlock until you complete the mission. It will not unlock immediately after getting a kill with your eighth weapon.

This mission contains far more than 8 weapons, and it's fully possible to get this achievement on Veteran difficulty. When the mission starts, choose any of the available four pistols; it doesn't matter which you choose. Proceed behind Price and kill the enemies outside. Pick up the pistol they drop, which will be your second weapon. You'll enter a building. Kill the enemies with that pistol. Grab the MP9 on the table and the gun one of the enemies drops. Head through the door into the bar area with the pool table. Get a kill with these two guns (your third and fourth) from behind cover when you enter. There are Uzis on the ledge where you enter, and another type of gun on the edge of the pool table. Grab these two and proceed on, getting a kill with each (fifth and sixth) in the next area. You should see enemies drop a P90 at this point, and another shotgun. Grab those and continue on. Kill the enemies outside with these weapons (your seventh and eighth). Inside the next building you can find another rifle, and then a FAL at the end of the next indoor section. You'll go outside then, and the Butcher will be caught. When you go inside the vehicle, you'll have an RPG and LMG to choose from. These are two more weapons, plus the FAL, that you can get kills with during this final section of enemies.

Once the mission ends, as long as you've gotten at least one kill with eight different weapons, the achievement will unlock.
Mission 13: "Going Dark"
Lights Out
Shut off the power to 4 buildings in 'Going Dark'.
Mission 13 "Going Dark" is the game's obligatory stealth mission, and offers some new gameplay elements, mainly the ability to shoot out lights, use night vision goggles at will, and a light meter to tell you how visible you are to the enemies. Additionally, you are given the opportunity to disable the power to buildings by flipping a power switch on the outside. There are four buildings where you can do this. Disable the power to all four to unlock the achievement. Due to the open ended nature of the "Find Hadir" objective, where you have to clear three buildings, you can do switches 2 - 4 in any order.

The following are the 4 places power needs to be shut off to earn this trophy:

1. Story related: Price will make you use the first switch to progress.
2. Pool: Go through the gate to the south (as if you were coming directly from where you rappel), it will be straight in front of you.
3. Clocktower: Directly across the street from a greenhouse.
4. Church: On the back side of the building next to the road, past the cemetery.

We Own the Night
Kill all enemies at the Church, Clocktower, and Pool without anyone calling for backup.
When you get to the main estate grounds, you'll be given three objective markers to investigate the Pool, Clock Tower, and the Church. For this achievement, you'll need to kill every enemy around the estate without an alert being raised. This doesn't necessarily mean you can't be seen, but you must kill the enemies before they yell out for help.

Turning off the power to all three buildings makes this far easier, since you'll be able to see enemies without them seeing you unless they're right next to you with their flashlights. Refer to Lights Out for disabling the power to each building. The killing of the enemies without alerts is very easy to do because there are a ton of checkpoints, and with the lights off it's really easy to see their flashlights coming from far away, and they can't detect you. The only annoying part is actually finding every single enemy in this area, because the achievement won't unlock until every enemy is dead. The ones inside each building are the most obvious and easy to kill. Just be aware that the second floor of the Clock Tower has a few enemies, and there's no reason to go upstairs except to kill those enemies for this achievement, so make it a point to do so. There is also an enemy outside the Clock Tower behind it, just standing there, so don't forget him either.

In general, sweep through every room of each building, and do a lap around the outside of every building, to ensure you don't miss any enemies. This achievement unlocks as soon as the last enemy is dead, not after the mission is completed, so don't interact with the hostage in the Church until you've cleared out all enemies and unlocked this achievement. If you get seen by the APC and it seems like enemies yelled for help, reload your checkpoint. Enemies yelling out in surprise when they see you does not void this achievement, only when they whistle and/or yell for backup.
Mission 14: "Into the Furnace"
Warheads on Foreheads
Reach Barkov's lab entrance using only drone strikes.
Right at the start of the mission, you'll go behind a tank and be given a laser to paint targets to call in drone strikes. You need to get to the top of the hill and into the door without firing a single bullet. I'm not sure if grenades of either type can be used, so it's best to avoid using them. You have unlimited uses of the drone strikes, but after four or five, you won't be able to use it for a minute or so until it reloads, so bear that in mind.

In general, hide behind the tank and only pop out to quickly paint a target. Keep this device equipped the entire time so you don't accidentally fire a bullet. Prioritize the helicopters that come in. If you destroy them quickly, it's less enemies on foot to worry about. Push up, and your allies will kill a lot of the enemies on foot for you. Be patient and don't rush. Let your drone reload and use it to take out all of the enemies so it's safe to move up. When you get to the top of the hill where the door is, once the last enemy is dead this achievement will unlock immediately.

SPECIAL OPS
Liberation
Complete all Special Ops missions.
As mentioned in the roadmap, there are four missions that need to be completed to unlock this achievement. Even though Activision and Infinity Ward have added additional missions via seasons, they are not required. Additionally, neither Survival nor Classic Special Ops are required.

The four missions are: Operation Crosswind, Operation Headhunter, Operation Kuvalda, and Operation Paladin. Each mission has its pros and cons, but both Headhunter and Paladin are relentless with their unending enemy spawns.
END
Tea Time
Complete the single player Campaign on any difficulty.
Self-explanatory

Out of the Fire
Complete every single player mission on Veteran or Realism difficulty.

Solid guide. Thank you!
For the Spec Ops achievement, in the end I had little trouble finding matches on a Saturday evening. Coming from Veteran, the missions aren't even that hard. I equipped the Medic role in case I need to revive players from afar, but didn't even get to use it once.

Only tip I have: don't get caught emptying your ammunition stock by reloading when you're prone. This will send you back to lobby with dev error 1099. My tip is to unbind all keys that allow you to go prone, if possible.

Unfortunately, I played Into the Furnace on Veteran and did not get Warheads on Foreheads. I'm fairly positive I didn't shoot any bullets, however, I did throw some grenades back so grenades for that achievement are a no-go I'm pretty sure. Even if you're simply throwing them back and didn't initiate the grenade yourself. :iAmTrash:
