Only heal spys and never anyone else. Even if they are burning before your eyes on the brink of death, the spy is more important. also if you see a team mate cloaking or disquising ALWAYS heal them. this is all you need to know to play medic.

BTW if you see a spy like this(Below), definitely heal him.

Used vaccinator to give a spy fire resistance. He killed all my team's pyros.
