There are 2 ways to proceed here with their own achievements, however if you want the 100% you need to follow through with the brain as the doll only gives 1

So 1st pick up the doll from toy box and give it to the therapist

Talk to your sister for a bobby pin
Now open this locker
Go back to your sister and enter the door next to her
Get chopsticks from the noodles
Hand the chef the fish and the mask

Go back to the end of hallway, dont go down but instead use your knife on the picture above
You might want to save incase you make a mistake in this next part
Enter and talk to the creature above. You have to give an item based on what they ask for. Useful = Chopsticks. Danger = Flower. Tradition = Fish
Leave this place and head to the final door next to us
Use the red tiles on the picture above pool
Now just follow the path to the end

Take the wood and make a bridge of it above
Enter the room
Get magnet on shelf above you
Exit the room and use magnet on pipe above
Use the pipe on the hole next to it
Click the flashing item to combine, then use it on the item
Go back into the room and use cable on the computer
The water is now drained and you can pick up another flashing item below
Use this key on your sister to leave

If you talk to all 4 people in this town you will get an achievement

Between the 3rd and 4th person you can see some stairs going down by some smoke. You need to go down them.
Here you can pick up an apple
Give this apple to the person sitting down for food
Go back to the save point and down
Open the gate and enter the left building
Now head past the apple man where a path leads up to the bathhouse again

Leave this room and start going right in the hallway.
A face will appear in the wall, if you enter it now you will die. However it did auto save so don't worry about saving first

Now make your way to the kitchen
Grab the knife from chef
Go to the locker room and talk to your sister
Use the knife
Go back to the chef
Grab the candy on the side

Story Achievement

Take the path up and to the right
We need to collect 5 mushrooms, there are 2 to the left of here. 1 we passed on the crossroad, 1 on the left path and 1 by the bus stop. There is a time limit and an achievement if you fail to collect and feed them to our sister in time. I suggest collecting all 5 mushrooms, saving and letting time run out. You do need to go to your sister to activate the death

Now actually feed them to the sister.

Get yourself some noodles here
We need to go back to the left pathway, where we picked one of the mushrooms
Make sure to hand over the noodles

Noodles will appease the spirit.

Hand over the mushroom to the big guy eating

Head all the way down and interact with the locked conductor door
Up 2 screens are some beds, enter the room that isn't locked off
Look in the side table
We can now go back down and open the door
Talk to the conductor
Now head all the way back up until you reach the sister
Talk to her for another keycard
Time to go further up the train yet again
Hand over the keycard, finally up one time to exit
Go back to the front and talk to the conductor

Luckily a teleport has opened up as we need to go to luggage
Push the red button here and go up the new ladder

Finally go back to the conductor and talk to them both

Head to the toilets on the right
Get pipe from sink
Go back to previous steps and to the left
Get candy bar from vending machine
Open window next to it with pipe
Use the computer
Head back to the elevators
Use candy on the open one
We can now go through the blocked gate before and go the die path
Go get the sister from the toilets
Now back to our ghost, giving her the doll

Before we take the live path on the right, exit this place and through the yellow police tape

We are at the end game here with multiple achievements. Once you know what to do its fairly simple repeating the steps from chapter select if you don't save at the right time or do certain steps out of place

This time don't go inside the bedroom but get the screwdriver from the draws outside it
Head into the bathroom
Open the hatch with that screwdriver and enter
Pick up the shovel to the side
Leave the house
On the right side of this house is a hidden knife to dig up

Go open the sisters door with the bobby pin
