They count through every run, so you can, say, get Naser's max affinity photo in one, then get all of Trish's in another, and they'll count for the achievement.
It's been a known issue, so past waiting for a patch I don't think you're gonna get much from the developer side. As for workarounds that aren't just using SAM to force unlock the achievement, I think if you edit your save to have all photos unlocked, then quit to main menu/close the game, it'll check and cause the achievement to fire? I haven't tested that, however, so YMMV.
AFAIK no, there's no way to figure out what photos you're missing/haven't counted yet. You'll have to guess at that point.
Probably both work. ALT+F4'ing during the following scene got it to count for me when I tried debugging that scene with an alt, anyway.
