Keep in mind the anomaly that you have found don't re-spawn until you find all the anomalies (took me only 4 gameplay to find all the anomalies) even if you fail. If you die/fail, the anomalies still don't respawn but the uncovered anomalies will get randomized.

Look at the ceiling above one of the stairs leading to the exit, and you'll see a shadowy figure

At the Don's Domain Exit, up the stair and right before the exit. Look at ceiling and there you'll find 2 Lights really close to each other

Basophobia

At the Blue Sign exit, one of the stair railing is missing and you only see the railing's shadow
