If the Poison Zombie is killed with hitscans or projectiles, if the killing shot hit the body, all of the headcrabs will detach intact and either skitter away or go after the attacker.
If the Poison Zombie is killed by explosives, the Headcrabs will all be blown off the body with varying amounts being alive or dead.
The extra headcrabs can be damaged and killed independently of the Zombie itself, with a hail of bullets being able to pick the Zombie's back clean of crabs.
