AI Ending 1: Win the game
AI Ending 2: Win the game in under 4-5 minutes.
AI Ending 3: Open the safe by using the map. Just look for the doll room and find the safe room (it’s the room where you need to use the code). Then, when you know what [ ] square the safe room is, just use the code shown on the map.

For ending 3, it seems that you don't necessarily have to do the safe room puzzle? I ended up unlocking ending 3 on my 2nd run by simply finding the syringe laying about on the ground.
