Report a translation problem
When you hit exactly 5 minute mark, you'll hear a loud lightning strike (I reloaded the save file twice and heard it both times)
If you still have any confusion about certain achievements then feel free to ask me about it.
Yeah I actually didn't know about the tragedy scene before trying to find out how to unlock it. I feel like it certainly is something that newer players will probably not achieve due to the nature of having to spam interact on a locked door.

Yeah this "remake/remaster" really does feel rough around the edges...
I just got all the achievements for Ai's story. Had alot of luck with the RNG I assume, cause I first found the syringe just lying there in my first playthrough and managed to get all items in a relative short time frame just a few runs after. So with 3 minutes, 534 steps and 2 deaths I still got Rank S.
As for the main story I think all achievements are fairly obvious, though you might miss the door and toilet ones if you play it for the first time.
Now on to get all the main story achievements...
